Gynecomastia is the enlargement of glandular breast tissue in men, and it is far more common than most people realize — commonly cited estimates suggest it affects a large share of men at some point in their lives, often during puberty, but also later in adulthood.

Glandular Tissue vs. Chest Fat

It is worth separating two things that get lumped together. True gynecomastia involves growth of actual glandular tissue behind the nipple, often felt as a firm, sometimes tender disc. Pseudogynecomastia, by contrast, is simply fat accumulation in the chest area with no glandular growth. The distinction matters because the two respond very differently to treatment.

What Causes It

The most common driver is a hormonal shift — specifically a change in the ratio of estrogen to testosterone. This can happen naturally during puberty (and often resolves within a year or two on its own), as part of normal aging, or as a side effect of certain medications, health conditions, or substances. In many cases, no single clear cause is ever identified.

What Doesn’t Cause It (Or Fix It)

Diet and exercise do not create true glandular gynecomastia, and once glandular tissue has developed, diet and exercise alone will not remove it — no matter how targeted the workout. This is the single most misunderstood part of the condition. If your chest is made of fat rather than glandular tissue, fat loss can genuinely help. If it is glandular, it generally will not, and understanding which one you are dealing with is the first real step.

What Actually Helps

For true glandular gynecomastia, surgical removal is the only treatment that eliminates the tissue itself. That is a real decision involving cost, recovery, and a conversation with a qualified surgeon — not something this article can tell you to do.
